Minor Release Form

Client Release Form

You must bring a state issued photo Id for minor, birth certificate and parent/guardian must bring state issued photo ID. We do require our minor release forms to be notarized. If you do not have access to a notary we have one at the shop! Minor must be (16-17) in the state of KY.

Failure to bring all the required documents will result in a forfeit of deposit and an automatic cancelation of the appointment

You must bring a state issued photo Id for your appointment. A paper copy is acceptable. We will not accept a photo of your ID it must be in hand at the appointment. Failure to bring required ID will result in forfeiting your deposit and a cancellation of your appointment.
